Key Responsibilities: The “Continuity of Care” Cycle
You are the primary anchor for the client’s daily independence, providing both clinical monitoring and lifestyle support.
AM/PM Care Routines: Assist with the critical transitions of the day, including waking, dressing, grooming, and preparing for bed.
Mobility & Ambulation: Support the safe movement and transport of clients, ensuring they can navigate their homes and communities safely.
Medication & Health: Provide medication reminders and follow a specific care plan to maintain the client’s health stability.
Domestic & Social Support: Manage meal preparation, light housekeeping, shopping, and errands, while providing meaningful companionship.
Clinical Documentation: Maintain accurate charting to ensure the entire care team—and the client’s family—are aligned on their current status.
Candidate Profile & Qualifications
