Job Summary

The Behavioural Personal Support Worker (PSW) at LOFT Community Services is a specialized “Clinical & Mental Health” role based in Toronto. Key Responsibilities: The “Behavioural Support” Cycle

You act as a bridge between clinical assessment and daily life, ensuring care plans are active and effective.

  • Primary Caregiving: Serve as the lead PSW for assigned seniors, delivering essential personal care while maintaining a therapeutic environment.

  • Behavioural Observation: Meticulously record “Responsive Behaviours”—identifying triggers, frequency, and intensity to inform clinical interventions.

  • Clinical Collaboration: Work directly with Psychogeriatric Case Managers and multidisciplinary teams to refine and execute personalized care plans.

  • Transition Advocacy: Support the high-risk periods when seniors move between care settings (e.g., hospital to home), ensuring continuity of support.

  • Peer Mentoring: Attend team meetings and provide guidance to colleagues, fostering a culture of shared expertise and professional growth.

Technical Insights: The Psychogeriatric Framework

Working in behavioural support requires a specialized toolkit focused on de-escalation and cognitive support.

  • Responsive Behaviour Mapping: You will likely use the ABC (Antecedent-Behaviour-Consequence) model to track and analyze why a senior is reacting to their environment.

  • Gentle Persuasive Approaches (GPA): You will apply techniques designed to respond respectfully and safely to challenging behaviours associated with brain change.

  • Cognitive Reserve Support: Implementing activities that leverage a senior’s remaining strengths to reduce agitation and promote “Quality of Life” metrics.

  • Multidisciplinary Sync: Your daily observations are the “Raw Data” used by Case Managers to adjust medications or environmental triggers.
