Key Responsibilities

Behavioral Assessment: Utilize clinical tools (such as PIECES or DOSS) to assess “Responsive Behaviors” and identify underlying physical or environmental triggers.

Care Strategy Design: Develop and implement individualized care plans that move beyond pharmacological interventions toward therapeutic, person-centred engagement.

Team Mentorship: Act as a clinical resource and coach for PSWs and fellow nursing staff, modeling best practices in dementia care and de-escalation.

Stakeholder Collaboration: Facilitate communication between families, physicians, and interdisciplinary teams to ensure a unified approach to resident well-being.

Quality Assurance: Monitor the effectiveness of behavioral interventions and adjust care plans in real-time to maintain a high standard of safety and dignity.

Medication Management: Oversee the administration of specialized medications, with a strong focus on reducing the use of antipsychotics in dementia care.
Candidate Profile

The “Clinical Mentor”: A Registered Practical Nurse (RPN) in good standing with the CNO, possessing at least 2 years of experience in Ontario Long-Term Care.
