Job Summary: CAT Case Manager (Youth Behavioral Health)

About the Role As a Community Action Team (CAT) Case Manager, you serve as the vital link between high-needs youth and the resources they need to thrive. Your primary mission is to optimize the functioning of children and adolescents with complex needs by coordinating medical, social, and educational services. This is a highly mobile, “boots-on-the-ground” role involving home visits and deep partnership with families. You won’t just manage a file; you will advocate for families, help them build natural support systems, and empower them to achieve autonomy in their homes, schools, and communities.
Essential Responsibilities:

Comprehensive Assessments: Evaluate individual youth strengths and family factors. Collaborate with families and providers to finalize assessments within 48 hours.

Home Visits: Conduct initial and ongoing home visits to evaluate client safety and well-being. Maintain at least 85% compliance with home visit requirements.

Service Planning: Partner with families to develop individualized plans with measurable goals. Update these plans every 3 months or whenever significant life changes occur.

Resource Coordination: Assist families in accessing financial, transportation, employment, and medical services. Build “natural support systems” to ensure long-term success post-program.

Clinical Documentation: Maintain meticulous records in the Avatar system, ensuring all progress notes are finalized within 48 hours of service and clearly link efforts to service plan goals.
Qualifications & Requirements:

Educational Background: Degree in a human services field (Social Work, Psychology, etc.) preferred to meet case management standards.

Technical Proficiency: Ability to utilize a laptop and navigate electronic health record (EHR) software (Avatar).

Communication: Exceptional interpersonal skills to build partnerships with youth, families, and external providers.

Mobility: Must be able to conduct frequent home visits across the service area.
