Website CommunitiCare Health

Job Summary

The Home Help Worker (Permanent Part-Time) at CommunitiCare Health is a vital community support role dedicated to helping clients in Toronto maintain their independence and dignity at home. As a “Supportive Pillar,” you provide the essential housekeeping and escort services that allow seniors or individuals with health challenges to remain comfortable in their own environments. This role requires the “wit” to observe and report changes in client well-being and the “grit” to perform physical labor and navigate the city between various client homes.

Job Details

  • Employer: CommunitiCare Health.

  • Location: Various client homes across Toronto (In-person).

  • Hourly Rate: $21.85 per hour.

  • Position Type: Permanent Part-Time (Existing vacant position).

  • Bargaining Status: Unionized (C.U.P.E. Local 4891).

  • Schedule: Monday to Friday, 9:00 am – 5:00 pm (No guaranteed hours/set schedule).

  • Start Date: April 1, 2026.

Key Responsibilities

  • Household Management: Perform light housekeeping, including vacuuming, dusting, sweeping, washing floors, laundry, and cleaning kitchens/bathrooms.

  • Nutrition Support: Prepare light meals, heat pre-cooked food, and assist with grocery shopping and garbage removal.

  • Client Escort: Accompany clients to medical appointments or shopping trips, assisting with carrying parcels and ensuring safe transit.

  • Vigilant Observation: Act as the “eyes and ears” of the clinical team by reporting any concerns regarding client health, safety, or changes in their living environment.

  • Team Collaboration: Actively participate in PSW/HHW meetings and training sessions to ensure high-quality, consistent service delivery.

  • Safety Compliance: Work in strict accordance with the Ontario Occupational Health & Safety Act.

Candidate Profile

  • The “Reliable Caregiver”: 1 year of housekeeping experience is preferred, with a genuine desire to serve marginalized communities and vulnerable populations.

  • Physical Grit: Ability to stand, bend, and kneel for extended periods, and the strength to lift 23 kilograms (approx. 50 lbs) independently.

  • Technical Wit: Strong time management skills to follow a fluctuating schedule and the ability to prioritize tasks based on individual client needs.

  • Certifications: Must have a valid Vulnerable Sector Check, a current CPR certificate, and proof of COVID-19 vaccination.

  • Communication: Excellent interpersonal skills with the ability to handle difficult situations involving families or clients with professional “Integrity-first” diplomacy.

Technical Insights: The Home Support Framework

Providing care in a private home requires a specific set of professional boundaries and safety techniques.

  • Infection Control: Since you work in multiple homes, you must master “Point-of-Care” sanitization to prevent the transfer of pathogens between clients.

  • Body Mechanics: To avoid injury while lifting 23kg or scrubbing floors, you will utilize ergonomic principles—lifting with your legs, not your back, and maintaining a neutral spine.

  • Scent-Free Environment: CommunitiCare is a scent-free agency; this is critical for clients with respiratory issues or sensory sensitivities (common in community health and addictions services).

  • The “Circle of Care”: Your observations are part of a legal framework. Reporting a “minor” change in a client’s mood or fridge contents can be the early warning sign that prevents a hospital readmission.
